This application is for a household budget. You must list all of your monthly bills and debts. Please fill out the top section completely for you and, if applicable, your co-applicant. In the center section, copy your income information from one of your normal paycheck stubs from each employer. List your day to day expenses (weekly and/or monthly) for items that are not regular monthly bills. In the third section, list all of your creditors, the balances, the amount of the normal monthly payment (if it were up to date) the amount in arrears (past due), and which month and day the next payment is due (if applicable.)

If the due date on a bill has already past, use the total amount due on the latest bill as the arrears, and put the upcoming bill's due date in the "Next Due Date" column. List the current charges as the normal payment. Use a second sheet  (or list in "additional comments" at the bottom of the online application) for additional creditors or comments regarding certain creditors, i.e. disconnect notices, right to cure default notices, etc. Please answer the questions that apply on the bottom of the page.

When completed, return it to us by fax, mail, or online. DO NOT SEND ANY BILLS AT THIS TIME. Within a few days after we receive your application, we will contact you with the information about your budget. If you have any questions, please contact us.
